2019 Contributing Sponsorship:

Contributing sponsorships include: One booth space with electricity, inclusion in all pre-event advertising, recognition in the event program, one-third page ad in conference program, logo on prominent signage in the meeting venue, company name and logo with link on conference website, lanyard sponsor (sponsor provides), option to provide attendee bag insert (GLEC only), Passport Program logo inclusion which gives attendees extra incentive to come to your booth (GLEC only), complete list of attendees following the event

Terms and Conditions:

Thank you for working with us to advance and support optometry in serving eye care needs in Michigan.

• Payment in full is due within 45 days of contract submission.
• Exhibit booth representatives due no later than three weeks prior to CE event.
• Exhibit booth includes, booth drapes (GLEC and MOA Fall Seminar), exhibitor identification sign (GLEC and MOA Fall Seminar), one skirted and covered six foot table, two chairs, and one electrical outlet (if indicated on contract). Fees quoted do not include special lighting, special signs, shipping or placement of display equipment. Multiple space exhibitors will receive the same amenities as a single space exhibit booth.
• Booths will be set up and coordinated by MOA Event Staff for Winter Seminar, Art Craft Display, Inc. of Grand Rapids for GLEC, Art Craft Display, Inc. of Lansing for Fall Seminar
• Registrants will receive via email an exhibitor kit and advanced shipping instructions after exhibitor registration forms are received.
• Booths will be offered to MOA Partner Program participants first, event sponsors second, and then on an "as received" basis for remaining exhibitors.
• Exhibit booth material, equipment, and literature must meet the professional standards of the MOA. The association reserves the right to prohibit any person from exhibiting if, in the opinion of the association, a display is not suitable nor in keeping with the character of CE.
• Lunches and coffee breaks for exhibitors are included in exhibitor registration fees.
• Exhibitors are responsible for staffing booths and security during exhibit hours.
• At the MOA Winter Seminar and MOA Fall Seminar Exhibit areas will be locked during non-exhibit hours.
• Security staff will be provided for evening hours at GLEC.
• Confirmation of exhibitor registration, booth floor plan, space assignment, set-up and tear-down hours, hall hours, and shipping instructions, will be emailed no later than 30 days prior to event.
• Exhibitors are responsible for making their own overnight room reservations. Room rates and promo codes can be found on www.themoa.org conference pages or www.glecmi.org.
• For good cause, exhibit space applications may be canceled, less a $50 administration fee, if notice is received by the Michigan Optometric Association not later than three weeks prior to event.

On behalf of company listed above in this contract (Exhibitor) I, the authorized representative, hereby agree to rent exhibit booth space and /or sponsor the Michigan Optometric Association (MOA) at the indicated events on the 2018 Michigan Optometric Association Exhibit and/or Sponsor Contract - A and will adhere to the under the terms and conditions of this contract.
